Procedent d'una família cosaca ortodoxa, Símon Petliura va néixer el 1879 als suburbis de Poltava, ara Ucraïna (llavors Imperi Rus). Va ser el tercer fill d'una família de nou infants, quatre germans i cinc germanes, i tres més que van morir en la infància.
