In Rebirth, there are 6 unique completion marks, and each one comes in two varieties, adding up to 12 marks in total. Afterbirth introduced 3 more, bringing the total to 17, as Greed mode only has one mark.

The Mark refers to the Mark of the Beast in the end of times prophecies (3 6's arranged in a triskelion). Without this mark, one would not be allowed to buy or sell. While The Mark's icon shows the 6's facing towards the center, on Isaac's forehead the symbol appears with the 6's facing outwards.
Apparently it's red for hard mode sp mark, blue for the online mark only, and purple for both. But its bugged af right now, creating random blue marks and actually deleting the completion marks from your SP save file because of it.
