Tras dos años en el conjunto inglés vuelve a Holanda para integrarse en el PSV Eindhoven, donde consigue ganar 3 veces la Liga holandesa y la Copa de Europa del año 1988. Continuó en las filas del PSV hasta 1994, año en que oficializó su retirada del fútbol.
After two years with Nottingham Forest, Van Breukelen moved to PSV Eindhoven in 1984. With PSV, he won six league titles and three domestic cups. In 1988, Van Breukelen also won the European Cup as part of the Treble after saving Benfica's sixth penalty in the shoot-out.

30 de abr. de 2024 · Als PSV kampioen wordt, reikt Hans van Breukelen de schaal uit. Die schone taak heeft PSV haar voormalig doelman en oud-lid van de Raad van Commissarissen toebedeeld. De 73-voudig international noemt het ‘een grote eer’ dat hij PSV officieel tot landskampioen mag kronen.
